DICTATION TECHNIQUES

At the beginning of each report:
  • Identify yourself.
  • Specify report being dictated; i.e. HP, Op, Discharge Summary
  • State patient's name and number, spell names
  • Give dates, i.e., admit, discharge, surgery date, consult date
  • Dictate your Attending, Resident and Intern for all cases.
  • Then dictate the report. Remember to enunciate and speak slowly.
  • Organize your thoughts.
  • Remember to use the correct work type; unsure call 8-6902.
ESIGN TECHNIQUES
  • Go into eSign at least twice a week and complete your entire list.
     
  • Sort list of documents by the appropriate column headers.
     
    • Sort list by TRANSCRIBED, by clicking on the column label. This will bring up your oldest reports.
       
    • Sort list by WORK TYPE, by clicking on the column label.
       
  • FOR ANY PROBLEMS in eSign, CALL 86180.
     
  • REJECTING A REPORT:
     
    • Click on the Edit icon to edit the report.
       
    • Type at the top of the report:
       
      • Enter two spaces, type asterisks (***), and type the reason why you are rejecting; i.e. wrong attending and give correct attending, wrong patient, etc. Then type more asterisks (***). EX: ***WRONG ATTENDING. CORRECT ATTENDING DR. SMITH***
         
    • Click on the Save icon to save the report. *Do Not Click on the Save and Sign icon.*
       
    • Click on the Reject icon to reject the report.
       
  • REPORT SEARCH –
     
    • When looking for a report by Medical Record Number. The number must be 7 digits. If you have a 6-digit number put a zero in front of the number. Always use leading zeros to get 7 digits.
       
  • PASSWORDS – Expire every 90 days, when prompted change your password. If you forget your password, call the I.S. Helpdesk at 86180.
     
  • INAUDIBLE MARK – Please look for the <…..> notation and correct the document prior to signing.
     
  • Resident Tab – Reports that are assigned to an Attending that have not been signed by Residents
     
  • Attending Edits Tab – Resident Reports that have been edited by the Attending Physician.
     
  • CC’s Tab – Reports that you have been copied on for distribution.
     
  • Logoff Tab – Always click this option when you are ready to exit eSign.
     
  • If you do not have your name at the top right corner of the screen, press F5 to refresh the screen.
